Multi National K9 booking application for the 2003 Christmas & New Year holidays.

Minimum 10 day booking over holiday period. Last date for dropping off your dog is December 24^th, 2003. MNK9 will be closed for pick up or drop off of dogs from December 25^th, 2003 {Christmas Day} to January 1^st, 2004 {New Years Day} & will reopen for pick up on January 2^nd, 2004.

Your $50 per dog non refundable holiday booking fee will cover;

#  Flee & Tick rinse hydro bath on your dogs entry to MNK9.            $10:00

#  Shampoo hydro bath on exit from MNK9.                                        $10:00

#  Tick collar for the protection of your dog for up to 2 months.           $15:00

#  All holiday surcharges.                                                                        $15:00

Extra daily fees apply for;

# Geriatric dogs         # Dogs requiring medical attention

# Administering veterinary approved medication.   # Special diets.

The balance payable is the daily Christmas boarding rate for your dog’s duration {Minimum of 10 Days board} and can be paid by cheque on drop off of your dog or by Efpos, credit card or cash on pick up.

Sorry no cheques accepted on pick up of your dog from MNK9.

MNK9 reserves the right to refuse your dog from boarding if you cannot provide proof of vaccinations or if the dog has suspected K9 cough, please ensure you bring in current Vaccination card on drop off of your dog.

Last date for boarding is December 24^th 2003. MNK9 reopens on January 2^nd 2004 for pick up of your dog{s}.

Opening hours Monday/Tuesday/Thursday/Friday 8 am to 10 am & 3 pm to 5 pm

Wednesday & Saturday 8 am to 10 am.

If you have to cancel or change dates please contact MNK9. If you don’t attend MNK9 on the date of your appointment your booking will be cancelled and the kennel will be rebooked at close of business the same day.
